The form is so we know about your past and current health and where you're going, therefore what vaccines you may need. By knowing this information, we can make sure you get your travel vaccinations on the same day.
When to book your travel vaccination appointment
Make sure you book your appointment for 4-6 weeks before your departure. Why? Some vaccinations take a while to become effective; others need doing over a few shots. Having your shots done 4-6 weeks before you leave increases your chances of protection.
However, we understand that you may have last-minute travel plans that have just popped up. Come to see us, and we'll accommodate you as best we can to help you have a safe holiday. Why are travel vaccinations needed?
Travel vaccinations are to protect you from any nasty diseases, such as Yellow Fever, that may spoil your holiday - the Australian government isn't too keen on you bringing something home also. Not every destination requires travel vaccinations, but certain countries do need them.
